Who can prepare meals, provide companionship, or offer a steady arm while walking through the house? PSWs assist with personal care and daily activities such as bathing, dressing, grooming, mobility, meals, and companionship. Ontario’s PSW training requirements include classroom learning and clinical placement, but PSWs are not regulated as nurses in Ontario.

Registered Practical Nurses

Ontario Families should also understand applicable WSIB requirements: domestic workers employed more than 24 hours per week by one employer are covered by WSIB, and employers of these full-time domestic workers must register. Care With a Clinical Component

Health assessments, certain medication-related needs, wound care, and other clinical services may require an RN or RPN. The appropriate provider depends on the Client’s condition, the care being performed, and whether regulated nursing knowledge and judgment are required.

When Nursing Supervision May Be Important

Nursing supervision can be particularly valuable when a Client’s health or care needs are changing or becoming more complex. Families may want to consider the level of clinical oversight available when:

  • Returning home after a hospital stay: Care needs may change throughout recovery and involve both personal assistance and clinical support.
  • Medication needs have changed: New or changing medication routines may require a review of what support can be provided by a Caregiver and when nursing involvement is appropriate.
  • Clinical care is required: Wound care, health assessments and other clinical needs may require care from an RN, RPN or another regulated health professional.
  • Mobility, cognition, or daily abilities are changing: Increasing assistance or noticeable changes may indicate that the existing care plan needs to be reassessed.
  • Several health and personal care needs overlap: Nursing oversight can help coordinate clinical care with the everyday assistance provided by a PSW.

Not every change means a Client requires direct nursing care. Nursing supervision provides clinical oversight so concerns can be reviewed and the level of support reassessed as needs evolve.

Personal and clinical needs often exist together. A Client might receive regular PSW assistance with mobility and personal care while also receiving nursing support for a health condition or clinical need.   • Which types of care are you comfortable providing? Discuss personal care, mobility, transfers, cognitive changes, meal support, and any other needs specifically rather than assuming they are covered.
  • How will care instructions be documented? Establish how routines, preferences, and changes will be recorded and communicated.

  • Who supervises and updates the care plan? Find out whether a Registered Nurse is involved and how the plan is reviewed as the Client’s needs change.
  • Are your Caregivers employees or contractors? Ask who handles screening, training, insurance, and employment responsibilities.
  • How do you match Caregivers with Clients? Experience should fit the Client’s care needs as well as their routines, preferences, and personality.
  • What happens if our regular Caregiver is unavailable? Understand how replacement coverage is arranged and who manages schedule changes.
  • How are changes in a Client’s condition handled? Ask what the Caregiver is expected to report, who reviews the concern, and how the Family is kept informed.
