30+ years of Nurse-Led Care
Across Canada, more families than ever are looking for ways to help their parents and grandparents age in place, safely, comfortably, and with dignity. Integracare has helped families do that for over 30 years, and today our teams serve Toronto, Mississauga, Ottawa and Winnipeg.
We provide personal support, nursing care, dementia and palliative services, physiotherapy, and more. Rather than coordinating multiple separate providers, families work with a single team that builds and manages one care plan together, every plan supervised by a Registered Nursing Coordinator, so clinical oversight stays consistent as needs change.

The Integracare Difference
Three decades of trusted, nurse-led home care across the Greater Toronto Area.
Providing in-home care across the Greater Toronto Area since 1991, with a track record across thousands of families and care situations.
Integracare is the only Toronto home care provider to hold the Dementia Friendly designation from the Alzheimer Society of Toronto.
We directly employ our caregivers rather than placing workers through a staffing agency, meaning consistent accountability, supervision, and standards across the team.
Every client is assigned a Registered Nursing Coordinator who provides active oversight around the clock, not just during business hours.
Clinical needs and personal preferences both inform caregiver selection. Coordinators aim for continuity, so clients see familiar faces, not a rotating roster.
PSWs, RPNs, RNs, Registered Massage Therapists, and Physiotherapists work together under a single coordinated care plan, not as separate, siloed referrals.

Excellent service. Kristine was extremely kind, caring and reassuring. I was stressed out with the state of my apartment and she put me completely at ease. Highly recommend this company!
Integracare supported my mom with her return home from a hospitalization, which was difficult given her dementia. They did an excellent job. And their caregiver was top notch. Thank you.
Thank you. Excellent care and service. I would recommend your service to anyone in need. 100%. Thanks again, Joan.

What families ask most about our home care services.
Integracare provides personal support from certified PSWs, clinical nursing care from Registered Practical Nurses, live-in care for clients who need round-the-clock support, and specialist dementia care. On the specialized side, we also offer palliative care, wound care, physiotherapy at home, and registered massage therapy. All services can be coordinated together under one care plan, supervised by a Registered Nursing Coordinator. Non-medical support, transportation, meal preparation, assistive device sourcing, and advocacy, is also available.
The free in-home assessment is designed to answer that question. A Registered Nursing Coordinator visits the home, reviews the client's medical situation and daily routine, and recommends the right combination of services. A family navigating a recent dementia diagnosis typically benefits from a mix of dementia-specialist care and PSW support; a client returning home after a hospitalization may benefit from RPN nursing combined with physiotherapy. Most care plans evolve over time, and we adjust the service mix as needs change.
Personal Support Workers (PSWs) provide daily living assistance, bathing, dressing, meals, mobility, companionship, and medication reminders. Registered Practical Nurses (RPNs) perform clinical tasks such as wound care, medication administration, injections, and clinical assessment. Registered Nurses (RNs) provide the highest level of clinical oversight, including care plan development and supervision of the broader care team. Integracare coordinates all three roles together under one care plan.
Call or contact Integracare to arrange a free in-home assessment. A Registered Nursing Coordinator will visit the home at no charge, assess the client's needs, and recommend a care plan. Once the plan is agreed upon, we match the client with a caregiver. Care can often start the same day you call, and intake is available 24 hours a day.
Costs vary depending on the type of caregiver, the number of hours, and the complexity of care. Visiting care is typically priced hourly, while live-in care is generally priced on a daily basis. PSW support and RPN nursing are priced at different rates reflecting their clinical scopes. Funding options include private insurance, employee benefits, veterans' benefits, and partial coverage through Ontario Health at Home for eligible clients. We can help families navigate available funding during the assessment.
Coverage depends on the plan. Many private insurance and employee benefits plans cover some or all of the cost of home nursing and personal support. Veterans' benefits programs may also provide coverage for eligible clients. Ontario Health at Home can provide partial, publicly funded home care for qualifying individuals, and private care from Integracare can supplement that coverage. Our coordinators can help families explore what applies during the assessment.
All Integracare caregivers are certified or registered in their disciplines: RNs, RPNs, PSWs, Registered Massage Therapists, and Physiotherapists. Integracare directly employs its caregivers rather than operating as a placement agency, so the team is hired, trained, and supervised to consistent internal standards. All client care is overseen by a Registered Nursing Coordinator, 24 hours a day. Integracare is also recognized by the Alzheimer Society of Toronto as a Dementia Friendly home care provider.
In most cases, yes. Care can often start the same day you call, with no waitlist. Integracare accepts intake 24 hours a day, 7 days a week. If a loved one's needs have changed suddenly following a fall, a hospitalization, or a shift in health status, call us directly and a coordinator will prioritize the assessment.

What families ask most about our home care services.
Yes, in most cases. Availability depends on your neighbourhood and the kind of help you need: hourly visits, overnight care, or a live-in arrangement are staffed differently. Share your postal code and what you're looking for, and our Winnipeg team will confirm availability before anything is arranged.
Families can continue receiving care through the Manitoba Home Care Program and use Integracare for additional care. For example, WRHA care may cover a morning visit while Integracare provides evening, overnight, or weekend care. Our nurse-led services include personal care from Health Care Aides, dementia and palliative care, companionship, help around the home and longer shifts, with 24/7 nursing support. We can also coordinate schedules with your case coordinator and confirm what can be arranged for your Family.
The Manitoba Home Care Program, through the Winnipeg Regional Health Authority, assigns hours based on a case coordinator's assessment. Many families keep their public home care and add private hours around it, for example, public coverage for morning personal care and our Health Care Aides for evenings or weekends. You don't need to choose one program over the other, and we can coordinate timing directly with your case coordinator.
The cost depends on the type of care and number of hours required. Nursing, personal care, occasional visits, and 24-hour care have different costs. Following a free assessment, we provide pricing based on your parent's care needs.
Yes. Our Health Care Aides are trained in dementia care, and a Registered Nurse supervises every care plan. Our dementia care in Winnipeg can start with a few hours a week and grow as your loved one's needs change.

All of Greater Winnipeg, from Tuxedo, River Heights and Fort Garry to St. Vital, Transcona, North Kildonan, St. James, East St. Paul and Headingley.
Yes. Call us before the discharge date and support can be in place for the first day home. Our nursing care in Winnipeg covers clinical needs after a hospital stay, and Health Care Aides help with mobility and personal care while strength returns.
Yes. Many adult children arrange care for a parent while living outside Winnipeg. Integracare also serves Families in Toronto, Mississauga, and Ottawa, with our home care services continuing to expand across Canada. We can begin by phone or video, involve your loved one in decisions about their care, and coordinate with Family members wherever they live.
Not much. Your parent's location, a sense of what's changed recently, whether it's mobility, memory, meals, or something else, and when you'd like to start are enough to begin. The deeper questions, medications, daily routine, and what matters most to your Family come later, in person during the free in-home assessment.
