In-Home Care Services

Integracare provides personal support from certified PSWs, clinical nursing care from Registered Practical Nurses, live-in care for clients who need round-the-clock support, and specialist dementia care. On the specialized side, we also offer palliative care, wound care, physiotherapy at home, and registered massage therapy. All services can be coordinated together under one care plan, supervised by a Registered Nursing Coordinator. Non-medical support, transportation, meal preparation, assistive device sourcing, and advocacy, is also available.

The free in-home assessment is designed to answer that question. A Registered Nursing Coordinator visits the home, reviews the client's medical situation and daily routine, and recommends the right combination of services. A family navigating a recent dementia diagnosis typically benefits from a mix of dementia-specialist care and PSW support; a client returning home after a hospitalization may benefit from RPN nursing combined with physiotherapy. Most care plans evolve over time, and we adjust the service mix as needs change.

Personal Support Workers (PSWs) provide daily living assistance, bathing, dressing, meals, mobility, companionship, and medication reminders. Registered Practical Nurses (RPNs) perform clinical tasks such as wound care, medication administration, injections, and clinical assessment. Registered Nurses (RNs) provide the highest level of clinical oversight, including care plan development and supervision of the broader care team. Integracare coordinates all three roles together under one care plan.

Call or contact Integracare to arrange a free in-home assessment. A Registered Nursing Coordinator will visit the home at no charge, assess the client's needs, and recommend a care plan. Once the plan is agreed upon, we match the client with a caregiver. Care can often begin within days of the initial assessment, and intake is available 24 hours a day.

Costs vary depending on the type of caregiver, the number of hours, and the complexity of care. Visiting care is typically priced hourly, while live-in care is generally priced on a daily basis. PSW support and RPN nursing are priced at different rates reflecting their clinical scopes. Funding options include private insurance, employee benefits, veterans' benefits, and partial coverage through Ontario Health at Home for eligible clients. We can help families navigate available funding during the assessment.

Coverage depends on the plan. Many private insurance and employee benefits plans cover some or all of the cost of home nursing and personal support. Veterans' benefits programs may also provide coverage for eligible clients. Ontario Health at Home can provide partial, publicly funded home care for qualifying individuals, and private care from Integracare can supplement that coverage. Our coordinators can help families explore what applies during the assessment.

All Integracare caregivers are certified or registered in their disciplines: RNs, RPNs, PSWs, Registered Massage Therapists, and Physiotherapists. Integracare directly employs its caregivers rather than operating as a placement agency, so the team is hired, trained, and supervised to consistent internal standards. All client care is overseen by a Registered Nursing Coordinator, 24 hours a day. Integracare is also recognized by the Alzheimer Society of Toronto as a Dementia Friendly home care provider.

In most cases, yes. Care can often begin within days of the free in-home assessment, with no waitlist. Integracare accepts intake 24 hours a day, 7 days a week. If a loved one's needs have changed suddenly following a fall, a hospitalization, or a shift in health status, call us directly and a coordinator will prioritize the assessment.
